9/13/10 N slapped another student’s neck, and ran around my classroom.

9/16/10 N slapped X’s head, messed with Y’s belongings, and repeatedly made paper “hornets” to shoot with rubber bands. I sent him to Ms. Z for a time-out.

9/23/10 When I redirected N, he said “ooh, you get on my damn nerves!”

9/29/10 N constantly sat turned to the side, talking and goofing off, and paying no attention. I told him to turn around at least 15 times. I finally sent him out of class.

10/8/10 N threw something at another student multiple times, and kept talking and yelling out as I tried to teach.

10/12/10 N had a huge issue with taking off his hoody when he arrived at school, defying and talking back to four teachers in turn, and our security guard. Then in class, N refused to do any work, called me a “dumbass,” got out of his seat repeatedly, ignored my instructions, and threw a pencil at another student. I tried calling home, but the number was “unavailable.”

10/13/10 N had sunflower seeds out and kept ignoring my instructions to put them away. He kept talking and shouting out while I tried to teach, and did no work for a long time. I eventually sent him to Ms. X for a time out, but he verbally refused to go, and wandered around the hallway. I then sent him to the Opportunity Room. I spoke with his grandfather about N’s behavior and failing grade, and he said that he would try to get a hold of N’s father. I also left a message at another number, including a request for attendance at next week’s parent conferences.

11/9/10 N kept getting out of his seat, standing up, wandering around, and sitting on top of desks. I told him to sit down 20 or 30 times before I sent him to the Opportunity Room.

11/11/10 N again showed that strange defiance about sitting down. He kept standing and wandering, and absolutely refusing to sit down. I calmly told him to sit down, and he calmly ignored me – dozens of times. I don’t know what the issue is.

12/8/10 N kept talking while I tried to teach, and then during work time he kept getting out of his seat over and over and over, completely ignoring all my redirections and warnings.

12/16/10 N absolutely refused to stop turning around and talking during our test. He received redirections, then progressively lost 20% from his test grade, then progressively got his three strikes, and was sent to the Opportunity Room.

1/4/11 X and N arrived in my room at 1:30 with a pass from the Opportunity Room signed at 1:05. Ms. Y confirmed that 1:05 was accurate, and they must have been wandering the halls for 25 minutes.

1/5/11 N yelled and cursed at another student. As I tried to teach he kept constantly turning around and talking, over and over. Then once we got to independent work time N kept constantly getting out of his seat, over and over, despite numerous reminders, until I sent him to the Opportunity Room.

2/18/11 N threw something at another student two different times. I went over to talk with N about how he has been working hard in class, but then when he does things like that it earns him strikes and seriously hurts his grade. But N refused to listen and was extremely disrespectful. I asked him to listen, and he said “nope,” and plugged his ears with his fingers. About fifteen minutes later I walked over to him to give it another try, and he lowered his head and said “I ain’t listening.” Then when we took our quiz at the end of class, N just sat there and did absolutely nothing.

2/25/11 N got out of his seat without permission as he’s been doing every day, talked while I tried to teach, and threw something at another student. I told him to go to the Opportunity Room, and he said “I ain’t going to the OP room,” and left. Ten minutes later he had not arrived there.

3/4/11 N was involved in throwing paper balls, pens, and pencils around the classroom. I caught him three times.

3/8/11 I caught N throwing paper balls in class.

3/9/11 N was throwing paper balls in class yet again! I called both the number on the shared drive and the number on PowerSchool, but couldn’t reach anyone.

4/14/11 The boys in this class had yet another paper ball and rubber band war, doing everything possible to disrupt class. I caught N throwing something, and sent him to the Opportunity Room.

4/15/11 The boys were yet again throwing paper balls and shooting rubber bands, and I caught N throwing a paper ball.

4/19/11 N put his head down and did no work, got out of his seat without permission, told me “you piss me off,” replied to my redirections and consequences by saying “I don’t care,” then threw something at another student, and I told him to go to the Opportunity Room. He said “You get on my fucking nerves,” and left.

4/21/11 N got out of his seat without permission, and was throwing things in class again.

4/29/11 N was throwing things in class yet again.

5/3/11 The boys in this class, who have such huge problems with throwing things and shooting rubber bands, already had issues with Ms. X today. She made them empty their pockets and roll up their sleeves in order to check for rubber bands. But then in my class I caught N with a paper hornet, took it, and warned him. Then I caught him with a second hornet, took it, and warned him. Then I caught him with a rubber band, took it, and sent him to the Opportunity Room. N angrily argued “ain’t nobody shooting fucking hornets in this damn room!”

5/13/11 N talked while I tried to teach, is constantly turned around in his seat refusing to pay attention and distracting other students, kept talking while we were taking a quiz, and kept talking back when redirected. I sent him to the Opportunity Room. He said “that’s fucked up,” and called me a “faggot ass.”

5/16/11 As we came in from recess, N kept jumping up and touching the fish art projects that are hung from the ceiling in the 7th grade hall. One fell, and I told N to take it to Coach X. N yelled back and argued, and said that someone else had knocked down the one that fell. N said “No, fuck that!” and refused to take the fish to Coach X. In class, N talked while I tried to teach, and did a ton of screaming and cursing at other students, especially L, including “shut the fuck up!”, “where’s my damn pencil?”, “fucked up hair!”, and “big ass head!”

5/18/11 N did a lot of cursing, talked back angrily and defiantly when redirected, as always he kept constantly sitting turned backwards so that he distracted himself and others from the lesson, and he stole K’s pen.

5/20/11 N kept constantly turning around and distracting himself and others, and refusing to face forward, and talking back when redirected. I moved him to the front row, and he did good work for a while, but then got frustrated and refused to work any more. He got out of his seat without permission, cursed, and talked back whenever redirected. Then he called X a “dirty slut.” He then called her a “dick-sucking ho,” and talked back when I redirected him. He said of X, “fuck her, I’ll slap her bitch ass,” and called her a “dirty ho.” I sent him to the Opportunity Room.

5/25/11 N kept turning around to distract other students, ignored instructions, talked back when redirected, and then during out big unit test, he kept turning around and talking until I told him to go to the Opportunity Room. He refused to go until I called security. He waited a while after I called, and then left before security arrived.
